JLPT special testing accommodations require an application through the relevant organiser, not a request made only on arrival.

The JLPT accommodations page explains support for candidates who have difficulty taking the test in the usual way because of illness, injury, disability or other conditions. Overseas applicants are directed to the local host institution; arrangements within Japan use the JEES process.
